feat: add opt-in Remotes web push notifications - #1138
Remotes alerts previously depended on an open page. Add Web Push for supported browsers and installed PWAs, with a separate Off-site notifications switch that is off by default. On-site alerts stay on by default. Existing OS permission does not count as opting in. Each account can enroll up to 20 browsers or installed PWAs; existing enrollments remain active at the limit.
Changes
Register and revoke account-specific delivery through authenticated, CSRF-protected endpoints; retain the origin-wide native subscription for reuse so delayed cleanup cannot break another account's enrollment. Only an explicit off-site toggle requests permission or creates a subscription. Rejected enrollments restore the previous choice. An uncertain enrollment attempts server revocation; if that cannot be confirmed, keep the explicit choice on and show an error so turning it off can be retried; stalled native operations recover after a deadline. A temporary passive registration failure preserves an existing enrollment for recovery on the next load.
Serialize browser push operations and off-site preference commits with one origin-wide Web Lock. Passive work reads current consent inside the lock, so a stopped or queued watcher cannot restore delivery after another tab successfully opts out. Lock acquisition and native/request operations have deadlines; browsers without shared locking cannot opt in. No additional stored consent generation is needed.
Queue one payload-free VAPID delivery job per subscribed recipient for new unread messages, rechecking membership, request status, mutes, blocks, and Remotes availability before delivery. The service worker shows a silent generic notification and opens Remotes without exposing sender or message previews to the push provider.
Reject a new enrollment at 20 with a clear browser error. An account row lock spans Form Request validation and creation, preventing concurrent requests from exceeding the limit. Existing endpoints can refresh at the limit, and revocation frees a slot. No ordering timestamps or automatic eviction are needed.
Coordinate enrollment, browser opt-out and sign-out with a PostgreSQL transaction advisory lock acquired before session loading and held through session saving. Protection lasts for the transaction, including slow enrollment; sign-out sees an enrollment that finishes first. Refresh Remotes consent under the enrollment account lock so a stale authenticated user cannot enroll after disabling Remotes.
Bind enrollment and opt-out requests to the account that initiated consent. Delayed permission, native subscription and queued work cannot enroll a newly signed-in account. A stale request also cannot revoke that account’s registration or unsubscribe its browser.
Bound each delivery attempt to one recipient and at most 20 subscriptions, with a 210-second job timeout covering the per-request deadlines. Skip recipients without subscriptions before loading their user records. Retry temporary failures only for the failed subscription IDs, preserving the queue/connection and the remaining three-attempt budget. Each retry rechecks current eligibility and enrollment.
Remove subscriptions when disabled, signed out, Remotes is disabled, the account is deleted, or the provider expires the endpoint. Restrict destinations to supported push hosts and never follow redirects.
Generate Wayfinder modules with form support before the Node unit command so a fresh checkout does not depend on ignored generated files or a production build.
Describe opt-in browser behavior in the existing product context and knowledge base; add feature, job, opt-in, recovery, and worker behavior coverage.
The new table stores only what delivery needs:

Validation
Required checks ran locally against an isolated PostgreSQL database. Unchanged provider/device evidence is identified separately:
composer test:lint — passed.
composer test:type-coverage — passed.
composer test:types — passed, including PHPStan and TypeScript.
bun run test:unit — passed: 170 Node tests.
bun x vp test run --config tests/draft-images.config.ts — passed: 528 component tests in 46 files. This suite also passed separately on the final revision. The Node SeriesStash regression invokes it as well.
Production asset build passed on the final revision. Composer audit previously passed on 44d8abbb; no dependency files changed since.
Real PostgreSQL concurrency check: starting at 19, two independent HTTP-kernel requests produced 21 rows without the enrollment lock; with the lock, responses were 204/422 and the count stayed 20. Feature tests also cover refresh at the limit, unchanged existing enrollments, account isolation, and freeing a slot.
Database-queue regression coverage reproduces the old duplicate sends (six requests instead of four), then verifies failed-only retries, exclusion of newly enrolled browsers, the shared attempt budget, and read/revoked/transferred enrollment checks. All 36 focused job cases passed, including two independently queued recipients with 20 browsers each and serialized timeout metadata; only the failing recipient/browser retries.
Real overlapping HTTP-kernel checks reproduced an enrollment surviving sign-out and another surviving Remotes disable. With the lifecycle fix, both leave zero subscriptions; the stale disabled enrollment returns 403. A twelve-second enrollment also drains before successful sign-out, crossing the former ten-second cache-lease boundary. Terminating an in-flight enrollment releases protection and allows logout to finish with no registration. An overlapping browser opt-out also waits for enrollment and leaves no registration. The 79 focused controller/session/job cases passed, including persisted-consent changes against a previously loaded authenticated user.
Account-switch regression coverage: five new Feature cases reject foreign/missing consent identities and preserve registration ownership. Client cases cover switching during permission/subscription/queued work, preserving an existing current-account subscription, and stale opt-out/failure cleanup. All 86 focused PHP and 70 client cases passed.
Two separate-module-realm regressions delay a successful opt-out or failed-enrollment cleanup response until another account has enrolled the shared subscription. Both retain that account’s working subscription and passive reconciliation. Rejected requests never delete a shared registration; uncertain network/server failures preserve the visible opt-in until revocation succeeds.
Two additional separate-module-realm cases use Node's real LockManager with shared fake native/server boundaries: passive work paused during native lookup or before POST commitment cannot restore enrollment after successful opt-out, even with all other channels off and its watcher stopped. These are not browser/device two-tab runs.
Fresh-checkout regression: removing the ignored controller module makes the original unit command fail with ERR_MODULE_NOT_FOUND. With all three generated directories absent, the updated command regenerated them and passed 168 Node and 525 component cases at the command fix. The final normal command passed 170 Node and 528 component cases after the additional consent regressions. No asset build is part of that command.
Actual Chrome service worker handled a DevTools-injected empty push after all localhost pages closed; the real showNotification call resolved. Transport was simulated; visible OS delivery is not claimed.
Visual evidence
The before/default/enabled screenshot matrix was refreshed against production base 5eae19d6e6a14c98e7bd512caca3876c4200aa6b and interface revision ea8a0670 (unchanged Remotes UI in final head db19efd84f22517d6e04f4cd79d7a6be177cdb53), using the same fictional account. The new limit-error captures use that interface. The final review base is 6ceabb0e8fee7f05f20f7a4775cf1c9867769843; the intervening upstream change only simplifies the separate Apps page and does not change any displayed notification control or its styles. Earlier non-limit recordings and native captures were carried forward where the displayed controls and behavior are unchanged; independent review revalidates them. Browser permission and PushManager responses are simulated; enrollment and revocation hit the real isolated Laravel endpoints. The failure recording deliberately returns HTTP 503. The expected local sign-in Inertia 409 is a login protocol response.
No horizontal overflow. Notification actions, sound toggles, polling, and client-side navigation produced zero measured layout shift; responsive changes in both directions kept overflow at zero. Initial page CLS reproduces on both base and head: 0.0035268004 at 1440×900, 0.0065741248 at 1366×650, and zero at 390×844. This existing page-shell shift remains outside this change.
Device verification
iPhone 18 Pro, iOS 27.0 simulator, Safari and installed Home Screen PWA. Safari correctly disables off-site enrollment and shows installation guidance. The PWA remains opted out even with OS permission already granted. Its real unavailable-configuration failure leaves off-site off and restores usable controls, with the toast above bottom navigation. Signing configuration was temporarily withheld during this check to prevent external Apple enrollment.
Touch, scrolling beyond the first screen, Safari controls, safe areas, fresh loads, client-side navigation, and local sign-in with keyboard dismissal were exercised. The changed switches add no keyboard-dependent input. No physical-device or Android coverage; Android SDK unavailable.
The server returns the actual capacity error; only browser permission/enrollment APIs are simulated. Off-site remains off and on-site controls remain usable. All six limit cases have zero measured page layout shift and no horizontal overflow.
The same error was exercised by touch in the iPhone simulator's installed PWA. Native permission/PushManager responses were simulated locally to avoid provider enrollment; the capacity rejection came from the real server. The temporary capture hook was removed before committing. The native clip trims setup and holds the final error for readability.
CODE: PASS · VISUAL: PASS — independent Sol 6.1 review of all 44 files at the exact base/head below. No actionable blockers. Independently passed 86 focused PHP cases (286 assertions), 70 client cases, four separate-module-realm cases and four worker cases. All 46 frozen media hashes match; actual screenshots and recording samples were inspected. Broad checks above were run by the implementer and their logs inspected by the reviewer. Historical concurrency and simulator evidence retains the provenance and limits described here.

The independent reviewer confirmed the unchanged presentation evidence applies to this final revision, including shared browser consent serialization, the backend queue partition and test-command changes. Earlier account-binding/shared-subscription behavior was independently reviewed at 690d202. Components, labels, styles, service worker and generic error-toast text are unchanged. An uncertain rollback uses the existing on-switch/error-toast presentation; this new failure path is covered through exported client behavior, without claiming a new device or provider run.
Limitations
A generic push already selected by a worker or accepted by its provider may still arrive after opt-out, sign-out or an account switch. These notifications contain no sender or message data.
Production signing keys and queue operation must be configured separately; this PR does not change production. Live provider enrollment/delivery, locked-device arrival, and successful native enrollment remain unverified. Browser/OS permissions and background execution affect delivery, including when a browser is fully quit. On iPhone/iPad, enrollment requires a Home Screen web app. Custom message sound only runs while the site is open. Queue delivery remains at least once: a lost provider response or worker interruption can still cause a duplicate despite failed-only retries.
Summary by CodeRabbit
New Features
Added optional off-site push notifications for new messages, with separate controls for on-site alerts, push delivery, and sound.
Push notifications require explicit opt-in and browser permission. They can appear while the app is backgrounded or closed, and open Remotes when clicked.
Added subscription management, including a limit of 20 browsers per account.
Exports now include push subscription IDs without exposing delivery endpoints.
Documentation
Clarified notification defaults, opt-in requirements, delivery behavior, and subscription limits.

Walkthrough
Priority: Normal
Change: Feature
Merge Risk: Minimal · up to db19e
Off-site Remotes push is opt-in, uses account-scoped and validated enrollment, and sends generic payloads with delivery-time eligibility rechecks. No open issues were found in this review. Live provider and device delivery should still be smoke-tested after deployment.
Security architecture risk: Low · up to db19e
The design separates explicit off-site consent from existing alerts and limits destination, account, and message-content exposure. A narrow privacy limitation remains: notifications already selected for delivery can outlive revocation or an account switch. They contain only a generic activity notice, not sender or message details.
Retained concerns
Low · security · inferred: A worker can load a subscription before revocation or account ownership transfer and send through that retained endpoint afterward. Session and enrollment locks do not fence this delivery path. Someone subsequently using the same browser could therefore receive a generic activity notification associated with the previous enrollment. The empty payload and generic display limit the inferred exposure to activity metadata, not sender identity or message content.
Security review details
Security Blast Radius
inferred — The new authority is outbound, VAPID-authenticated delivery to enrolled browser endpoints. Enrollment is bounded per recipient account, not globally across all message-triggered jobs. Providers receive endpoint and delivery-timing metadata but no sender or message payload. The service worker displays a silent generic notice and restricts click navigation to the application origin.
Security Findings and Attack Paths
inferred — The retained concern requires a revocation or ownership change after subscription selection but before transport or browser delivery. It affects the browser retaining that native subscription and reveals only a generic activity signal. The inspected path does not provide message access, sender details, or account impersonation.
Trust Boundaries and Controls
observed — Attacker-supplied endpoints pass through HTTPS provider-host validation before persistence; credentials, fragments, non-443 ports, and root paths are rejected. Redirects are disabled at transport, constraining the server-side request destination.
observed — Mutations use authenticated web routes and same-origin requests carrying an XSRF token. Both request classes reject a captured account identity that differs from the authenticated account. Deletion also filters by authenticated owner and endpoint hash, so stale cleanup cannot directly delete another account's enrollment.
Resilience and Maintainability Implications
observed — Delivery retries retain failed subscription IDs and a decreasing attempt budget, then reapply current recipient and ownership checks. Connection failures preserve enrollment; provider 404/410 responses remove it. The transport uses a ten-second request timeout and a 300-second provider TTL. These controls bound stale work but do not cancel already-selected or provider-accepted delivery.
Hardening Proposals
proposed — Define the revocation cutoff explicitly, including notifications already selected or accepted by a provider. If strict account-switch isolation is required, introduce an enrollment-generation or equivalent display-authorization mechanism rather than treating row deletion as cancellation of in-flight delivery.

SendRemotePush loads each SQL-eligible participant before checking subscriptions. Users without subscriptions can therefore still incur user loading and block or mute queries. Add whereHas('user.pushSubscriptions') to remove this avoidable work.
The effect is bounded by the 100-member conversation limit, so this is an optional performance improvement, not a major queue issue.

Addressed the summary’s account-switch concern in 71a94132 and 690d2024. Enrollment and revocation carry the initiating account ID; the Form Request compares it with the authenticated account before any ownership change. Delayed permission, subscription creation and queued work cannot apply A’s consent to B.
Revocation now removes only the account-owned server registration and retains the origin-wide browser subscription for reuse. This also fixes a delayed-successful-DELETE race: A’s response cannot unsubscribe a subscription B enrolled in another tab. If an uncertain enrollment cannot be confirmed removed, the switch stays on with an error so revocation can be retried.
Independent CODE/VISUAL: PASS at 690d2024; 84 PHP, 67 client, two cross-realm and four worker cases independently passed. Required local checks passed: 4,361 PHP tests at 100% coverage, 168 Node tests, 525 component cases, lint, types and build. Live provider/OS delivery remains unverified. Major · Split the recipient loop into one job per recipient. A single job… · SendRemotePush.php:52-65
app/Jobs/SendRemotePush.php:52-65 🩺 Stability & Availability | 🟠 Major | 🏗️ Heavy lift
Split the recipient loop into one job per recipient. A single job timeout cannot bound the whole loop.
handle() sends to every eligible recipient and every subscription in one job, with no send cap. Each push can wait up to 10 seconds. Two recipients at the 20-subscription cap can take about 400 seconds in one attempt. $timeout = 210 covers only one recipient at the cap, so later recipients can still be lost.
If the worker stops the job during the loop:
Recipients after the stop point receive no push.
The follow-up dispatch after the loop never runs, so failed IDs are not retried.
The job stays reserved and runs again after retry_after (960 seconds). By then the message is older than the 5-minute check at the top of handle(), so the retry returns without sending. The unsent recipients get no notification, and no failure is recorded.
The loss needs a worker timeout shorter than the loop duration. The repository does not set one, and Laravel's queue:work default is 60 seconds.
Correction: dispatch one SendRemotePush per eligible recipient, carrying that recipient's user ID. Each job sends to at most 20 subscriptions. A $timeout of 210 seconds then bounds each attempt. Keep the failed-ID retry inside the per-recipient job, and update the CreateMessage dispatch and the tests to match.

Addressed the outside-diff recipient-budget finding and the summary’s same-account opt-out race in bb6e4806 and db19efd8.
Delivery now queues one job per subscribed non-sender recipient, snapshots at most 20 subscriptions, and sets a 210-second job timeout. Failed-only retries keep that recipient, failed IDs and the remaining original three-attempt budget. Real database-queue tests verify two 20-browser recipients require two separate worker deliveries, with timeout metadata and recipient isolation.
Browser enrollment, revocation and off-site preference commits now share one origin-wide Web Lock. Passive work reads current consent only after acquiring it. A stopped watcher cannot POST after another tab successfully opts out; queued work sees the committed off preference. Acquisition/native/request deadlines remain bounded, and browsers without shared locking cannot opt in. No extra persisted consent generation or server state was added.
The same-account race was reproduced before the fix. Two regression cases use Node’s real LockManager and actual source exports in separate module realms, pausing native lookup or POST commitment with on-site alerts and sound off and the watcher stopped. Both retain server revocation after successful opt-out. Native/server boundaries are faked; no browser/device two-tab replay is claimed.

Reaching the 20-browser limit
The server returns the actual capacity error; only browser permission/enrollment APIs are simulated. Off-site remains off and on-site controls remain usable. All six limit cases have zero measured page layout shift and no horizontal overflow.
Browser MP4
The same error was exercised by touch in the iPhone simulator's installed PWA. Native permission/PushManager responses were simulated locally to avoid provider enrollment; the capacity rejection came from the real server. The temporary capture hook was removed before committing. The native clip trims setup and holds the final error for readability.

Independent review
CODE: PASS · VISUAL: PASS — independent Sol 6.1 review of all 44 files at the exact base/head below. No actionable blockers. Independently passed 86 focused PHP cases (286 assertions), 70 client cases, four separate-module-realm cases and four worker cases. All 46 frozen media hashes match; actual screenshots and recording samples were inspected. Broad checks above were run by the implementer and their logs inspected by the reviewer. Historical concurrency and simulator evidence retains the provenance and limits described here.
